When you use our services, you’re trusting us with your information. We understand this is a big responsibility and work hard to protect your information. This Privacy Policy is meant to help you understand what information we collect, why we collect it, and how you can update, manage, export, and delete your information. How We Collect Information

Advanced Regenerative Therapy and its employees and volunteers collect data through a variety of means including but not necessarily limited to letters, phone calls, emails, voicemails, and from the submission of data and forms that are either required by law or necessary to provide you with quality care.

When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection. An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy. After approval of your comment, your profile picture is visible to the public in the context of your comment.

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

By entering your email address, mailing address, contact phone number (land line or cellular device); you expressly agree to receive marketing updates and communications from Advanced Regenerative Therapy by email, voice, SMS, mail, or social media. All communications can be terminated by either unsubscribing from our email list via the unsubscribe link, replying to SMS messages with the term “stop”, verbal request by calling 678-926-3834, or in person at our office located at 4889 Golden Parkway, Ste. 110, Buford, GA 30518.

Cookies

If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

How We Do Use Your Information

If you apply or attempt to apply to receive care through us and provide information with the intent or purpose of fraud or that results in either an actual crime of fraud for any reason including willful or un-willful acts of negligence whether intended or not, or in any way demonstrates or indicates attempted fraud, your non-medical information can be given to legal authorities including police, investigators, courts, and/or attorneys or other legal professionals, as well as any other information as permitted by law.

What We Do Not Do With Your Information

Information about your financial situation and medical conditions and care that you provide to us in writing, via email, on the phone (including information left on voicemails), contained in or attached to applications, or directly or indirectly given to us, is held in strictest confidence.

We do not give out, exchange, barter, rent, sell, lend, or disseminate any information about clients or patients who apply for or actually receive our services that are considered patient confidential, is restricted by law, or has been specifically restricted by a patient/client in a signed treatment and HIPAA consent form.

Who We Share Your Data With

If you request a password reset, your IP address will be included in the reset email. With the exception of legal requirements and visitor comments, your data will never be shared with anyone by us.

Legal Reasons

We will share personal information outside of Advanced Regenerative Therapy if we have a good-faith belief that access, use, preservation, or disclosure of the information is reasonably necessary to:

  • Meet any applicable law, regulation, legal process, or enforceable governmental request.
  • Enforce applicable Terms of Service, including investigation of potential violations.

Visitor Comments

Visitor comments will be checked through an automated spam detection service.

How Long We Retain Your Data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

What Rights You Have Over Your Data

If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

Exporting, Removing, & Deleting Your Data

You can contact us and request that we deliver to you, remove, and/or delete any and all data collected from you at any time. Alternatively you may export or delete your data directly from Google.

Limited Right to Use Non-Identifying Personal Information From Letters, Notes, and Other Sources

Any pictures, stories, letters, biographies, correspondence, reviews or thank you notes sent to us become the exclusive property of ART. We reserve the right to use non-identifying information about our clients (those who receive services or goods from or through us) for fundraising and promotional purposes that are directly related to our mission.

Clients will not be compensated for use of this information and no identifying information (photos, addresses, phone numbers, contact information, last names or uniquely identifiable names) will be used without client’s express advance permission.

You may specifically request that NO information be used whatsoever for promotional purposes, but you must identify any requested restrictions in writing. We respect your right to privacy and assure you no identifying information or photos that you send to us will ever be publicly used without your direct or indirect consent.
